Locally the cause is normally one of: an old clay pipe joint that's finally failed, a pitch-fibre pipe from the 1950s to 70s that's deformed and eventually collapsed under the weight of the ground above it, or tree roots that have forced a joint apart bit by bit over years until the pipe gives way.
I confirm what's actually happened with a camera before any digging starts, so I know exactly what I'm dealing with rather than guessing. I open up only as much ground as the job needs, usually a single trench over the damaged section rather than the whole run, replace or repair the pipe properly, then reinstate the surface, whether that's concrete, tarmac or slabs, as close to how I found it as I can.
